What happens if I cannot make it to a scheduled appointment?
Please give us 24 hours notice. A $100 charge will apply for missed appointments without 24-hour prior notification.
I need a refill on my prescription. Do I need to come in?
Often there’s no need for an appointment. Simply contact the pharmacy where your prescription was previously filled and let them know that you need a refill. They will contact our office electronically for the refill approval. If your prescription needs to change or was prescribed for a short duration, such as to treat an infection, your practitioner may ask you to schedule a follow up appointment before a refill is given.
What insurance do you accept?
LSFP takes most medical insurance coverage. We accept:
- Medicare
- Cigna
- Tricare
- Aetna
- Anthem
- Carefirst
- United Healthcare
We also accept other small insurance carriers but please call the provider’s office before making an appointment.
Do you accept new Medicare patients?
Yes, a few providers at LSFP are still accepting new Medicare patients. It is critical that all Medicare beneficiaries utilize their wellness-care benefits and recommended preventative care in order for LSFP to continue to participate in the Medicare program and for patients to access all benefits.
Can you provide directions to the office?
Maps for all LSFP locations are available under Contact Us.
What is your fax number?
